Sematic is software for practices that want to operate APCM with their own team. It organizes the work while your practice retains the patient relationship and continues making clinical, operational, coding, and billing decisions.

Sematic follows the APCM work your team already does: enroll the patient, build the plan, follow up, document activity, and bill the month. Teams receive onboarding and work from a guided workspace instead of reconstructing the process across spreadsheets, chart review, and billing queues.

Sematic brings patient review, coverage and tier context, consent and enrollment, care-plan creation, follow-up work, panel management, documentation, claim readiness, and authorized bulk submission into one workflow. It supports the work; it does not replace the practice's responsibility to meet APCM requirements.

Both. New programs can establish enrollment, care-plan, follow-up, and month-end routines during onboarding. Existing programs can bring their current panel and map established workflows into Sematic. Implementation timing depends on the practice, its athenahealth configuration, and whether the program is launching or migrating.

Sematic can organize patient data, draft care-plan content, surface follow-up suggestions, and prepare work for review. Authorized staff decide what is clinically appropriate, what is saved to the chart, and which claims are submitted. Sematic assists the care team rather than making autonomous clinical or billing decisions.
